I have a form created. How do I make the submit button send the form to my e-mail address?

Save the form source code in html format and attached it to your email. When you send it out, your friend would be able to view the form when opened under the html browser.

It's somewhat difficult to have a string HTML code send something through an e-mail. If you want to do that you need to create another script page, in your case, probably a javascript page. I would suggest a program that does it for you like...
